PRE-ENROLLMENT

How do we enroll in the web service?

If you do not have an account:

Please visit our homepage (www.dmv.ny.gov/lens) and follow the Pre-Registration link at the lower left of the page to apply.

Upon receipt of this information, we'll email you a user id and mail a password to the mailing address on file with your driver license.

If you have an existing account:

Send an email to LENS with the following information:

  • Your 5-digit LENS customer number,
  • Your driver's license number,
  • Your business telephone number, and
  • Your business email address.

Upon receipt of this information, we'll email you a user id and mail a password to the mailing address on file with your driver license.

Do we need software to use LENS?

No special software is necessary. What you use to browse web pages and read email should work.

Can we use both LENS and Dial-In?

Yes. However, the programs require separate applications. They can both use the same escrow (billing) account.

I recently applied to use LENS. Will I receive confirmation?

Yes. We'll email you an account number and user id and mail a password to the address on file with your driver license.

We sent in a LENS application. When can we expect a reply?

Enrollment usually takes less than two weeks. Contact us if you have waited longer.

Why do you collect my driver license number before assigning my LENS user ID and password?

We require positive identification before granting access to LENS.

We recently pre-registered for LENS. When can we expect a response?

LENS will email you immediately. Please review the email for further instructions.

After we receive our welcome email from LENS, must we supply additional information to use the service?

No. Our welcome email includes instructions on using LENS.

Why do you mail my LENS password to my home?

Mailing ensures that you are the only person to see the password. It is the only piece of LENS correspondence we send to your home.

Do we need a signed release from a driver to enroll him/her on our roster?

No. However, you do need a permissible use listed in the Federal Drivers Privacy Protection Act (DPPA). You indicate your permissible use(s) on your LENS application.

What are the fees to participate in the program?

Who is exempt from LENS fees?

Government agencies, volunteer ambulance companies, and volunteer fire companies using LENS for a public purpose.

Are there annual fees?

No.

ACCOUNT MAINTENANCE

How long does our LENS account remain open?

Until you close it.

How do we update our contact information in LENS?

Follow the "Modify/View Customer Profile" link from your LENS main menu. Correct your information and select the "Update" button to save your changes. If you're changing the contact person, email LENS with the new contact's driver license number.

We have misplaced our customer number. Could you send it by email?

Please contact the LENS Help Desk.

I have misplaced my LENS password. How do I retrieve it?

Send an email to LENS with your user id. We'll mail a new password to the mailing address on file with your driver license.

I have locked myself out of LENS. What should I do?

email LENS with your User id and your LENS customer number. We'll mail a new password to the mailing address on file with your driver license.

Can more than one person use our LENS account?

Yes. Provide the following information for each additional user:

We'll mail the password directly to the new user. Please note, regardless of the number of users, LENS will provide notifications only to the contact person.

What if an employee with access to our LENS account leaves our organization?

Please send an email to LENS with the name of the employee and your LENS account number.

LIST MAINTENANCE

Are transactions done by phone, email, or US mail?

All transactions can be completed from the LENS web page.

How do we revise our roster?

When we enroll drivers in the LENS program, will we be notified of any current suspensions or convictions that appear on their records?

Yes. When you enroll a driver into LENS, we will send you an abstract of the driver's record. The abstract will show the convictions and suspensions that are in effect.

As I add drivers to LENS, I see the driver's license status before I finish adding him/her. Am I charged to view this information?

Yes. New York State law requires the DMV to charge for searches of its records. Each search costs $7. If you are using LENS as a pre-hire screening tool, we recommend that you add any driver that you look up. You can later drop the driver if you choose not to hire.

Is there a limit to the number of drivers I can add to LENS?

The LENS website allows you to add drivers one at a time with no limit. If you wish to add or drop a batch of drivers, please make arrangements with the LENS Help Desk.

Is it possible to get a list of the drivers we have enrolled in LENS?

Follow the "Order Driving Records" link on your LENS main menu. Without entering any data in the available fields, choose the "search" button. LENS will display the drivers currently on your roster

When we add drivers to LENS, will we receive abstracts for these drivers?

Yes.

Is it possible to get a driving record of someone already on our roster?

Yes. Follow the "Order Driving Records" link from your LENS main menu, enter the license number, customer index, or the last name of the driver, and select "Search". Choose the blue circle to the left of the driver's name. Choose the "Order Driving Record" button to request a driving record. Driving records are $7 apiece.

We are interested in the notification history of a driver who we earlier enrolled in LENS. Can we obtain this information?

Yes. Follow the "Recent Transaction Information" link from your LENS main menu and enter the license number or customer index of the driver into the spaces. You will see a list of all notifications LENS has produced for the driver.

Does LENS automatically generate an annual driving record for each driver on our roster?

No. LENS generates a driving record when you add a driver. For additional records, follow the "Order Driving Records" link on your LENS main menu.

NOTIFICATIONS

How are event notifications sent?

LENS emails notifications to you.

How does LENS send driving records?

LENS produces certified paper driving records and mails them to you.

What type of information will I find on a driving record?

Please visit Form DS-242.1 http://www.dmv.ny.gov/forms/ds2421.pdf to view a sample of a driving record.

A month ago, I received a notification that one of my drivers was suspended. Yesterday, the driver brought a receipt from court stating that they had resolved the suspension. Why have I received nothing from LENS?

Some events reach DMV more quickly than others. LENS can only notify you of an event after it posts to the driver's record. If a driver provides you with a disposition from a court and you haven't seen corresponding information from LENS, you may confirm by contacting the court that produced the disposition.

Why are we notified of a driver's failure to answer a summons, but not notified that the driver had received a summons?

LENS does not notify of summonses (tickets). A summons is not a public record unless, and until, there is a conviction.

One of my drivers was in an accident yesterday. I am signed up to receive notifications of accidents. Why didn't LENS inform me of this event?

An accident report must be indexed at the DMV before it is recorded on a driver's record. Some accidents take longer than others to index. LENS will notify you of the event after it posts to the driver's record.

What information is contained in a conviction notification?

Conviction notifications contain the conviction date, violation, violation date, serial number of the ticket, and point value of the conviction.

I have a conviction notification, and wish to determine whether my employee was driving his own vehicle or my company's vehicle at the time of the violation. Can LENS tell me?

Not directly, but you can obtain a photocopy of the traffic ticket. Tickets usually show the plate number, place, time and date of the incident, which may be useful for your purpose. Order a photocopy of the ticket using Form MV-15 http://www.dmv.ny.gov/forms/mv15.pdf and include the ticket serial number noted on your LENS notification.

We haven't received any notifications from LENS in the last five or six weeks. Can you verify that our account is still active?

After you have enrolled drivers in LENS, your account is active until you close it. If you don't receive any notifications, it means that no new events have been posted to your drivers' records.

BILLING

Must we keep a minimum balance in escrow?

No. Just keep enough in your account to meet your immediate needs.

Can I pay by credit card?

No.

OTHER STATES' RECORDS

I really like the LENS Program. Are you planning to expand it to other states?

The LENS program can only notify you of events on New York State driver records. For information from other States' records, you will need to contact the other States.
