Organ and Tissue Donations

For information about organ, eye and tissue donations through the New York State Donate Life Registry, visit the Organ and Tissue Donation page external link at the NYS Department of Health Web site.

You can enroll in the Registry online through MyDMV.  (Note: Sign up information from a driver license or other photo document issued by NYS DMV and other personal information is required.)

You can also enroll when you complete the donation section on a driver license or Non-Driver ID renewal application form or on form MV-44 (Application for Driver License or Non-Driver ID Card) and submit the form to a DMV office.
