NYS Organ & Tissue Donor Enrollment	Use to enroll in the New York State's Organ & Tissue Donor Registry. This form is available from the NYS Health Department.
Applications to Open Default Conviction - AA-3.3	Use this form to request that the DMV reopen a Traffic Violations Bureau traffic ticket (from New York City, Buffalo, Rochester, or Suffolk County (with a violation date before April 1, 2013)) that resulted in a default judgment against you. You must include any documents that may be necessary to support your statements, and you must be willing to participate in a hearing. This form cannot be used in local courts or for tickets where a default judgment was not entered. Traffic Violations Bureau Appeal Form - AA-33	Use to file an appeal from a conviction and/or penalty imposed for non-criminal moving violations after a DMV Traffic Violations Bureau hearing in NYC, Buffalo, Rochester or Hauppauge (with a conviction date prior to April 1, 2013) only (see Traffic Tickets). For convictions or penalties by courts in other locations and courts, contact the court, not DMV, for appeal information. Determine if you can file your appeal online instead of using form AA-33.	2 pages
Appeal From DMV Hearing Determination - AA-33A	Use to file an appeal from a finding and/or penalty imposed after a DMV hearing regarding such matters as a fatal accident, chemical test refusal, or motor-vehicle related business regulated by DMV. Also used to appeal denial by DMV of driver license or business permit application. Cannot be used to appeal suspensions or revocations resulting from traffic violation convictions in a court or a DMV Traffic Violations Bureau.	2 pages

Driver Training Programs Complaint Form - DTP-201	Use this form to send a complaint to NYSDMV about any of the following: a driver education program or instructor, a driving school or instructor, a point/insurance reduction program delivery agency, sponsor, or instructor, or a pre-licensing course instructor. There is no fee to file this Complaint Form. Note: To send a complaint to NY DMV about a vehicle repair, inspection or purchase, use form VS-35.	2 pages
Physician's Request for Driver Review - DS-6	Used by a licensed physician to notify the DMV of a driver licensed in NYS when the physician determines that the mental or physical condition of the driver can affect the safe operation of motor vehicles. Request for Driver Review - DS-7	Used by a person who is not a physician or a police officer to notify the DMV of a driver who has a possible medical condition that can affect the safe operation of motor vehicles. Request for NYS Driving Privileges - DS-115	Used by out-of-state drivers to request restoration of their New York State driving privileges following a revocation of privileges. This form cannot be used to request restoration of a NYS driver license or learner permit following a revocation. Revoked NYS drivers must apply for a new license using form MV-44.	1 page

Claim for Credit of Sales Tax Paid to Another State (For Motor Vehicle, ATV, Vessel (Boat) or Snowmobile) - DTF-804	Used when a New York resident purchases a vehicle out-of-state, registers the vehicle in New York State and applies for credit for the sales tax paid in the other state.	2 pages
Application for Refund and/or Credit for Sales or Use Tax Paid on Casual Sale of Motor Vehicle - DTF-806	File this form with the NYS Department of Taxation and Finance to claim refund or credit for payment of vehicle sales tax or use tax.	1 page
Affirmation Under Section 318 (9) and 318 (11) of the New York Vehicle and Traffic Law - FS-15	Must be filed with license application after a mandatory one-year revocation for an uninsured accident has been served and mandatory $750 civil penalty paid. This form required only after a revocation for uninsured accident.	1 page
Request for Insurance Information for NY Registrants Involved in an Accident - FS-25	Use this form to have DMV determine insurance information for New York registrants on the date they were involved in a specific motor vehicle accident.	2 pages
Financial Security Exemption Application - FS-48	Used to apply for an exemption from NYS auto liability insurance coverage requirements (NYS Financial Security Law) while a vehicle was at a registered NYS repair shop or junk/salvage facility. Form must be completed by the shop or facility and signed by the manager of the shop or facility and the vehicle registrant.	1 page
Financial Security Exemption Application - Storage - FS-48S	This form is used to apply for an exemption from NYS auto liability insurance coverage requirements if coverage lapsed while a vehicle was stored at a commercial or military facility.	1 page
Certificate of Removal From United States and Canada - FS-113	Two copies included. Filed in duplicate with registration receipt when a vehicle registered in NYS is being removed from the US and Canada and will be exempt from NYS insurance requirements.	1 page

Withdrawal of Consent - MV-1W	Used by a parent or guardian to withdraw consent for a driver license or learner permit issued to a driver under age 18. When consent is withdrawn, the driver license or permit is cancelled. Consent is granted on form MV-44 or MV-44S.	1 page

Statement of Abandoned Vehicle - MV-37	Use to transfer a vehicle at least ten model years old and worth less then $1250 from a property owner to a vehicle dismantler, itinerant vehicle collector or certified scrap processor when the vehicle has been abandoned for at least one month.	1 page

Statement of Ownership for Non-Titled Vehicles, Boats, Snowmobiles and All-Terrain Vehicles - MV-51B	Use to certify ownership of a non-titled vehicle, vessel, snowmobile or all-terrain vehicle and to certify why proof of ownership is not available. Certificate of Employment - MV-58A	Use to certify employment of junior driver. Completed by the employer of a junior driver. The junior driver carries the form when permitted to drive to and from work without a parent, guardian, driver education teacher or driving school instructor. Odometer and Damage Disclosure Statement - MV-103	For use by a dealer or private owner where the title or dealer's certificate of sale does not have an odometer/damage disclosure statement section required by law.	1 page

Certification of Supervised Driving - MV-262	Every driver license applicant who is 16 or 17 years old at the time of the road test, and who has a junior learner permit, must present this certification to the DMV license examiner at the time of the road test. Transfer of Vehicle Registered in Name of Deceased Person - MV-349	Use to transfer the vehicle of a deceased person when there is no will, estate, surviving spouse or surviving minor child, and the vehicle is worth $25,000 or less. For information see Deceased Vehicle Owner.	1 page
