Use these instructions to replace a photo document by mail (driver license, learner permit, or non-driver photo ID card). You can replace a document by mail if you are in another US state or another country.

Send the completed form to the address shown on this page. Include the copies of your proofs of identity and a personal check or a money order to pay the fee.
These instructions are for replacement of a regular driver license, learner permit or non-driver photo ID card with a regular document. You cannot convert from a regular document to an enhanced document by mail.
If you replace your document 40 days or less before your 21st birthday, you will be issued a new document without the "UNDER 21" logo. But the new document will arrive in the mail close to your birthday. However, if you replace your document at a DMV office an interim document can be issued to serve as acceptable proof while waiting for the new document to arrive in the mail.
Read the instructions (form MV-44.1) and complete the application (form MV-44). You must respond to all the questions on the back (page 2) of the form. Make sure that you sign your name in the space indicated on form MV-44. If you do not know your Client ID number, copy exactly your full name and your date of birth from your photo document.
Copy your proofs of identity and send the photocopies with your application. Get information about proof of identity. Note: Form MV-44.1 indicates that photocopies are not acceptable, but photocopies are acceptable if you send the application by mail. If you lost your photo document, you must send photocopies of other proofs of identity that equal a value of six points. If you do not have proofs of identity that equal a value of six points, you must go to a DMV office. Bring all the proofs of identity that you have to the office.
You can have your document sent to an address that is different from the address on your document. Include an additional note and clearly print the alternate address. Do not write the alternate address on form MV-44. The mailing address you write on form MV-44 is the address that is listed on your photo document and your DMV record.
If your current document displays the words, "VALID WITHOUT PHOTO", but your photo does not appear, call the DMV Call Center to get more information.

The DMV can deliver your photo document by express mail. Your application must include a self-addressed express mail envelope that is pre-paid for delivery.
