A Private Service Bureau (PSB) license is required of any person, firm, association or corporation engaged in the business of assisting for hire in securing licenses to drive motor vehicles, registrations, or titles of motor vehicles.

How to Apply for a Private Service Bureau License

Approved Name

Before you can apply for a license to operate a PSB, the name of the PSB must be approved by the DMV. To avoid public confusion, a name that includes "DMV" or "Motor Vehicles" will not be approved.

You must first submit form PSB-3 (Name Selection Form for PSB Applicants) and receive approval from the DMV.

Application Requirements

To apply for a license to operate a PSB, you must submit to the NYS DMV:

  • Your form PSB-3 that shows your PSB name was approved by the DMV (see "Approved Name" above).
  • A photocopy of a valid NYS driver license or non-driver photo ID card for the business owner(s) and all employees of the PSB.
  • A completed Form MV-372 (Application for a License to Operate a Private Service Bureau or Open a Branch Office)
  • A completed Form MV-521.1 (Personal History) for each owner, partner, corporate officer, agent, employee and major stockholder (20% or more of stock) listed on Form MV-372.
  • A copy of the receipt provided to clients as required by Section 77.5 of the Regulations of the Commissioner of Motor Vehicles. See Form PSB-2 (Sample Receipt Form).
  • If you do not have employees, you must submit a completed Form CE-200 (Certificate of Attestation of Exemption from NYS Workers Compensation) available from the NYS Worker's Compensation Board. This form is to show a government agency that NYS Workers Compensation insurance is not required.
  • If you have employees, you must submit your proof of current Workers Compensation and Disability Insurance Coverage.
  • A Business Certificate (X-201), a Business Certificate for Partnerships (X-74) or NYS Department of State Filing Receipt (DOS-1025)
  • The application fee of $25.00 (non-refundable). Make checks payable to Commissioner of Motor Vehicles.

What to expect after your application has been received/reviewed

After your application is reviewed by the DMV in Albany, it is sent to your local Motor Vehicle office. The local office schedules an inspection. When the inspector visits your location, the inspector can ask you to provide the following for review:

  • The original lease
  • The original business certificate and/or corporate papers and/or filing receipt
  • A sample customer receipt
  • A permanent, bound book
  • A filing cabinet
  • Your proof of Workers Compensation Insurance (if required)
  • Three character reference letters.
