Send photocopies of your NYS photo driver license or other proof of identity that equal a value of six points or more. Do not send your original proofs of identity.

You can have your registration items sent to an address that is different from the address on your registration document. Include a separate note and clearly print the alternate address. Do not write the alternate address on form MV-82.

The weight of your vehicle determines the amount of your fee. The total amount of your fee can include a fee for custom plates or a vehicle use tax. Your registration fee normally does not change:
Pay your fee by personal check or money order payable to "Commissioner of Motor Vehicles".
Important: Fees that are incorrectly calculated cause delays. If you do not know the correct fee, make your check payable to "Commissioner of Motor Vehicles", but leave the amount blank. The tear strip receipt that you receive with the registration documents will show the total amount of your fee.
Send all your documents and your payment to the address shown below. Your registration items will arrive in two weeks. If you do not receive the items within two weeks, contact your DMV Call Center or visit a DMV office. Your items will not arrive as quickly by international mail.NYSDMV Port Jefferson Office
